Outside of the above dates, the West Office (Door N3) will be open for work permits and transcripts on the following Wednesdays from 9:00 am until 11:00 am - June 17, July 1, July 15, July 29. Thank you!
Click for the Minor Work Permit . All sections must be completed before it can be processed. Part 1 must be filled out by the student and family, and Part 2 is to be completed by the employer. Part 3 requires a physician’s signature; however, an existing current sports physical on file may be used instead.
